How to Provide Vaccination Records

The Dog House requires proof of rabies, DHPP, canine influenza, and bordetella vaccines.
Vaccines must be administered by a licensed veterinarian.

Behavior evaluations may not be scheduled until valid vaccine record have been provided to our office.

After you've provided your pet's vaccination records, a member of our staff will process them and contact you with instructions on how to schedule a behavior evaluation for your pet.
Records must be processed manually; to ensure timely review, please do not upload them multiple times.
